Diabetes is a serious chronic health condition that can affect many aspects of your health and wellbeing. It can increase the risk of heart disease, stroke, kidney disease and peripheral vascular disease. The condition and its day-to-day management may also influence a person’s mental and emotional wellbeing. Some people may experience stress, worry, or frustration, while the demands of monitoring blood glucose, taking medications and making ongoing lifestyle adjustments can also feel overwhelming at times.
However, having the right knowledge, support and guidance can help you feel more confident in preventing, managing the condition and reducing the risk of complications.
Mr Albert Yeap, a pharmacist and credentialled diabetes educator, provides personalised and evidence-based diabetes education at Valewood Clinic for people living with diabetes, as well as those who may be newly diagnosed or at risk of developing diabetes. Education is tailored to your individual circumstances, health needs and goals, with practical strategies that can be incorporated into everyday life included.
Diabetes support may include education regarding diabetes technologies (i.e. continuous glucose monitoring, insulin pumps), insulin injections, healthy eating and nutrition, your sleep patterns for optimal quality and medications.
Working closely with our GPs and other healthcare professionals, Albert’s diabetes education service forms part of a coordinated, patient-centred approach to diabetes care and better long-term health outcomes.
Whether you are currently living with type 1 diabetes, type 2 diabetes, gestational diabetes or are at risk of developing this condition, Albert is ready to provide clear, practical and compassionate education to equip you with the knowledge, resources and skills to achieve your health goals.
Consultations are bulk billed with a valid GP Chronic Condition Management Plan (GPCCMP).
Mr Albert Yeap is available at Valewood Clinic on Saturdays monthly.
